Breast Cancer Screening Challenges in Palau

KOROR, Palau — A recent report from the Palau Hybrid Survey 2023 has spotlighted significant gaps in breast cancer screening and non-communicable disease (NCD) management in the Pacific island nation. The findings underscore the urgent need for increased awareness and consistent healthcare access to improve early detection and treatment for women.
Since its launch in 1998, the Palau Breast and Cervical Cancer Early Detection Program has played a pivotal role in offering mammogram screenings for women aged 40 to 74. According to Selma August, Non-Communicable Disease (NCD) Manager, the program has screened 6,777 women as of December 2023.
Despite these efforts, the report reveals that only 64% of women aged 50-74 have undergone a mammogram within the recommended two-year timeframe. This falls short of the program’s goal of improving early detection rates. August reiterated the program’s message: “Early detection is your best protection.”
Screenings are available weekly on Thursdays from 9 a.m. to 11:30 a.m. and Fridays from 9 a.m. to 11:30 a.m. and 1 p.m. to 4 p.m., yet many women remain underserved. The report highlights that women outside the eligible age range are particularly affected, as mammograms are not recommended for them. Instead, clinical breast exams are advised only if a physician deems them necessary.
The survey also paints a broader picture of Palau’s health challenges. Among adults with chronic conditions, only 24% of individuals with diabetes and 21% with hypertension have their conditions well-controlled. Additionally, cancer screenings across the population show a significant shortfall, with 84% not screened for colon cancer and 35% not undergoing cervical cancer screening.
To address these gaps, the Ministry of Health is urging eligible women to utilize the available resources and prioritize regular screenings. Raising community awareness about the importance of early detection and expanding access to health services are critical steps in improving outcomes for women.
